COD Student COVID-19 Vaccination Status Verification

Per the Executive Order, students who are not fully vaccinated will be required to receive their first dose of the Pfizer or Moderna vaccine or single dose of the Johnson & Johnson vaccine and adhere to weekly COVID-19 testing until fully vaccinated (see below for details).

All students taking courses on campus will be required to verify their vaccination status via a secure two-step process:

  1. Submit intake information.
  2. Email an image of your vaccination card.

To complete Step 1:

  1. Log into the student portal,
  2. Click on myACCESS for Students, and under the Miscellaneous header click on the Submit My Vax Info link.
  3. Enter your info and click Submit.
  4. You will receive a system email that your submission has been received.

To complete Step 2:

  1. Using your COD email account (, attach an image of your Vaccine Card and send to Please note that any submission not using the email account cannot be processed.

    The software used to collect vaccine information can read .jpg, .jpeg, .pdf and .gif file formats. Other file formats such as HEIC or shared files such as Sharepoint cannot be opened and verification of your vaccine information may be delayed.

  2. You will receive a system email that your submission has been received.
  3. Please note: Your submission will still need to be approved which may take up to 48 business hours. You can check your clearance status in your myACCESS account under Academic Profile, under my profile.
