COD Student COVID-19 Vaccination Status Verification

Per the Executive Order, students who are not fully vaccinated will be required to receive their first dose of the Pfizer or Moderna vaccine or single dose of the Johnson & Johnson vaccine and adhere to weekly COVID-19 testing until fully vaccinated (see below for details).

All students taking courses on campus will be required to verify their vaccination status via a secure two-step process:

1. Submit intake information.
2. Email an image of your vaccination card.

To complete Step 1:

  1. Log into the student portal,
  2. Click on myACCESS for Students, and under the Miscellaneous header click on the Submit My Vax Info link.
  3. Enter your info and click Submit.
  4. You will receive a system email that your submission has been received.

To complete Step 2:

  1. Using your COD email account (, attach an image of your Vaccine Card and send to Please note that any submission not using the email account cannot be processed.
  2. You will receive a system email that your submission has been received.

Student Vaccination Cash Voucher Incentive

College of DuPage is implementing a vaccination incentive for students who received their vaccinations after the Sept. 9 announcement from the College regarding compliance with the Governor’s Executive Order. To qualify, students must upload their vaccination card showing full vaccination by Monday, Nov. 29 indicating they were vaccinated between Sept. 9 and Nov. 29. Eligible students will receive cash vouchers of $150 in early December via direct deposit on a first-come-first-serve basis.

NOTE: At this time, all vaccination incentives have been distributed to students who submitted vaccination records by Nov. 29 with a vaccination that occurred between September and Nov. 29, 2021.
