Here's where you register your institution as a downlink site for a College of DuPage teleconference series. There are two ways to register:

1. On Paper:
   Pay by check or credit card number, or have us invoice you with or without a purchase order. Simply print out our 1-page registration page. (You'll need a free Adobe Acrobat Reader to view it.) Fill it out, then mail or fax it, as you prefer. Addressing information is on the page.
2. On Line:
   Have us invoice you for the teleconference series with or without a purchase order number. Simply scroll down this page to "Register Now!", type into each of the boxes below, and click on the 'Register Now' button.

• $385 USD if you register before November 7, 2003
• $485 USD if you register after November 7, 2003

Registration is for your entire site. If an individual would like to attend the series, they should contact your local downlink site directly.If you are a student seeking college credit for Soaring, you need to register with the college.

We will send you, as Site Coordinator, the coordinates for tuning into the Soaring to Excellence satellite broadcasts approximately one week before each teleconference.

Normally, we send this information by two different services (e.g. E-mail and/or FAX and/or U.S. Mail). If you wish the information sent to anyone else at your institution (e.g. a technical contact person) or wish to identify someone else as Site Coordinator or a back-up Site Coordinator, please list their information in the box marked "Additional Contacts." As web-based materials for each teleconference become available, we will notify the Site Coordinator by e-mail.
