Transcript Evaluation Request Form
FOR INCOMING TRANSCRIPTS ONLY
**An official transcript must be on file in the Office of Student Records for each school you are
requesting to be evaluated prior to submitting this form. Verify receipt of your transcript(s) in your myAccess account under Academic Profile.**
- DO NOT SUBMIT THIS FORM if you have not registered for classes at College of DuPage.
- DO NOT USE THIS FORM if you have petitioned for a degree or certificate.
- DO NOT USE THIS FORM if you have applied to a health science program.
Please note:
- Incomplete request forms will delay the processing of your request.
- Processing time for Transfer Evaluation is approximately 6 to 8 weeks.
- Evaluations for new students do not begin until two weeks into the first term in which the student is enrolled.
- Results of the Transfer Evaluation are emailed to address indicated below.
- To check the status of your evaluation, log onto http://myaccess.cod.edu, click on "ACADEMIC PROFILE", then select "MY PROFILE." The status will be listed under "TRANSCRIPT INSTITUTIONS."
