Eligibility Requirements
• Full-time, part-time and temporary non-bargaining unit classified employee

• Employment by the college for at least one year

• Anyone who has not won the award for at least six quarters

• Not under threat of suspension or termination by written warning

Instructions
1. Complete the criteria checklist. Check off all that apply.

2. On a separate sheet, explain in your nomination why your candidate should be considered for this award. Do not include the name of your candidate in your typed nomination.

3. On another separate sheet, in 30 words or less, summarize your nomination. This will be used for publication if your candidate is selected.

4. Complete and sign the transmittal form.
