Deadline:
November 1, 2009

Purpose:
To assist students pursuing a health care career who have been accepted into or enrolled in a certificate or associate degree program in Nursing or Allied Health at COD

Requirements:
• Admitted to or enrolled in a certificate or degree-granting program in Nursing or Allied Health
• Resident of District 502
• Submit paragraph validating scholarship need
• File 09-10 FAFSA. COD must receive information electronically by deadline date.
• Submit one letter of recommendation from program coordinator

Terms:
• To be used for tuition, fees and books beginning Spring semester 2010
• $1,000 scholarships will be offered annually. Students in a two-year program may re-apply for the second year
