of the community with dedicated volunteers for charitable organizations
within DuPage, Kane, Kendall and Will Counties

Purpose:
To provide financial assistance for students pursuing a career that
will contribute to the social welfare of our community, including human
services, health care, the ministry, and other related professions
Requirements:
• Enrolled minimum 12 credit hours per semester
• Maintain cumulative 3.0 GPA
• Resident of DuPage, Kane, Kendall or Will counties
• Document participation in volunteer services at least 30 hours
per year
• Must be pursuing a degree in the field of human services, health
care, the ministry, or a related service field
• Submit statement on career goals, volunteer experience, how
and why you became involved
• File 2010-11 FAFSA. C.O.D. must receive information electronically
by deadline date
Terms:
• To be used for tuition, fees and books and related
educational expenses at C.O.D. beginning Fall Semester 2009
• May be renewed for second year if criteria are met
