Please describe the purpose for your request. Note:

  • This form is only to be used for minor updates such as date and hyperlink changes to online courses.
  • Please print hard copies (with URL at top) of your course pages with changes indicated and attach to form. Send to Cathy Rathke, Manager of Web & Instructional Technology.
  • For IT assistance, this form must be submitted three weeks before the end of the term.
