Use this form if you wish to have software or data files installed or modified in any of the academic labs. Return the completed form to the Manager of Office and Classroom Technology, Information Technology. Instructions and Printing Directions

(1)If this is a software package, you must include the software license agreement along with this request. The license agreement must authorize the College of DuPage to install the total number of licenses you are requesting.
