to use the form:
This is a request
to format or author a DVD with a menu that includes chapter points.
To request this service you must have rights to the
media you wish authored, you must provide the media to Multimedia
Services in an appropriate format, which would include digital files,
DV digital video tapes, simple pre-recorded DVD's or high quality
videotape recordings (VHS). You should not use this form
to request a simple (no menu) DVD or DVD duplication; use the Multimedia
Duplication
Request form instead.
