2014 DuPage Menís Soccer Team
(last modified 6/7/14)

Student/Athletes wishing to participate on the 2014 College of DuPage menís soccer team need to complete the following tasks:

Note - Students trying out for the soccer squad are encouraged to take the following summer course (not required):
PHYS-1161-030- Physical Fitness I - Physical Fitness for Men’s Soccer, with instructor Jim Kelly, 1 credit hour. Class meets every Monday, Wedneday and Thursday evening from 5:00 - 7:00. Class begins June 30th and finishes August 3rd. This course will provide a transition from the off-season to pre-season.

Step 1 - Complete the Intercollegiate Athletics Eligibilty Packet [IAEP]. These packets are available in the Athletic Office [PE] or from any member of the coaching staff. Included in the packet are eligibility forms, an academic information sheet, and various medical forms. All forms must be completed and returned before a player can be declared eligible to play. It is strongly recommended that the forms be completed and handed in at the pre-season meeting. Medical forms are available at the bottom of the collegeís team page: www.cod.edu/athletics/ppe_packet.pdf

Step 2 - Have a physical examination by a licensed physician. No athlete will be allowed to tryout unless they have proof of a physical (a physical form is attached to the IAEP). Physicals must be dated on or after July 1, 2014 to be valid.

Step 3 - Attend the team meeting on Monday, July 28th at 1:00 pm The team meeting will be held in the Physical Education Building - room 129.

Step 4 - Practice/tryouts began on Friday, August 1st at 1:00 pm, therefore any new players would need to contact a member of the coaching staff prior to showing up. See contact information below.

Practice/tryouts are for all players new and returning. Players are required to bring soccer shoes, running shoes and shinguards. Water will be provided. It can not be emphasized enough that players show up to the first practice physically fit for college soccer. Practices are Monday through Friday at 1:00 pm but are subject to change. Practices rarely last longer than 2 hours. There may be some double or Saturday sessions before games start.

Any additional questions, contact: William Fajkus, Assistant Menís Soccer Coach at (630) 942-2479. Coach Fajkusí office is located in the PE Building, Room 202c, and his summer term hours are M-Th from 8:00-2:00; every day class is in session.
