PPO Plan Members

Are your children graduating from College?  If so, they will not be eligible as a dependent under our PPO plan as of their graduation day!   Let us know if you have a dependent that will be graduating, so that we can offer them continuation of coverage under COBRA.  Dependent children are covered over the age of 19 to their 24^th birthday, provided that they are a full time student at an accredited school/university.  Contact Sue Benton at x 2427 or Gail Conidi at x 4272 for more information. 

HMO Plan Members

Dependent children are covered to age 24 under the HMO plans, provided that they are dependent upon you for support and unmarried.  They do not need to be a full time student to be covered under the HMO medical insurance plan.

Note:  Blue Care Dental, Guardian First Commonwealth DMO and Vision Service plan all require full time student status over the age of 19.

Vision Benefits

Vision Service Plan has announced new benefits enhancements effective 7/1. 

Signature Plan

• 30% off unlimited additional pairs of prescription glasses when purchased the same day as the member’s eye exam from the same VSP doctor who provided the exam

• 30% off unlimited non-prescription sunglasses when purchased the same day as the member’s eye exam from the same VSP doctor who provided the exam

• New and current contact lens wearers are eligible for a covered in full contact lens evaluation and initial supply of approved lenses from our newly expanded list, including toric, multifocal, and hydrogel lenses.

If you have a dental plan through COD, then you automatically have Vision Service Plan vision benefits.  Go to www.vsp.com and check out your eligibility or call them at 800-877-7195.,

Moving in your plans this summer? 

Please contact Human Resources at x2640 and we will send you out forms to change your information on our system.  You can change your address with Blue Cross Blue Shield (both HMO and PPO plans) at www.bcbsil.com.
