Finally, it appears that spring has arrived! With it comes these reminders:

April 30th is the final day to submit a claim for your 2007 Flexible Spending Account. Medical Reimbursement claims can be submitted for any eligible service provided through March 15th 2008. Remember, the service had to be provided by that date. Claim forms are available on DAC's website at www.dacadmin.com. To log on to their site, our log in is COD, password is college. You can also stop by HR to pick up a form or call DAC at (630) 773-2337 to ask for a claim form.

Is your child graduating from College this Spring?

If so, then they will not be eligible for the PPO plan as of their graduation date. Contact me at x 2427 or Gail at x 4272 to get information on COBRA ( how to continue their health insurance under COBRA rules) and the forms to take them off of your insurance.

Dependent children are covered until their 24th birthday, provided they are a full time student, under the PPO, Predent or DMO and Vision Service Plan. HMO Illinois and Blue Advantage HMO consider dependent children up to age 24, even if they are not a full time student but still dependent upon the employee for support, as eligible under the HMO plan.

Do you need a replacement Blue Cross Blue Shield card? You can go to www.bcbsil.com and register on that site to request a replacement card or even print out a temporary card. Once you are registered, you can receive emails to let you know that claims have been processed and check out your medical and dental benefits.
