Any claim must first be filed with the student-athlete's personal medical insurance. If there are any balances due after payment has been made by the student-athlete's primary insurance, those bills, along with the Explanation of Benefits should be submitted to the Athletic Trainers. The college and/or its athletic insurance company will consider paying the balance based on usual and customary charges. There are, however, limitations to this coverage: Medical bills must be submitted to the College of DuPage Athletic Trainers within 30 days or the student-athlete assumes financially responsible for those charges. Student-athletes must have received evaluation, treatment, and/or rehabilitation from a COD Athletic Trainer. The athletic insurance policy provides payment for usual, customary, and reasonable charges incurred within 1 calendar year following the date of injury. The athletic insurance applies only to accidental injuries sustained during participation in intercollegiate sports or supervised travel pertaining to such activities. The athletic insurance does not provide coverage for: sickness or disease in any form; medical or hospital expenses to treat an illness or injury that is not a result of practice for or participation in Intercollegiate Athletics at College of DuPage. Injuries sustained in Physical Education classes are not covered by the Athletic Department. If additional coverage is desired for sickness/disease, or for coverage outside of Intercollegiate Athletics, supplemental insurance may be purchased, for an inexpensive cost, through Student Health Services, IC 2001, (630) 942-2154. As is the case with all medical insurance, certain costs are not covered under this insurance policy. Any costs not covered as a result of policy exclusions must be borne by the student-athletes. CLAIMS PROCEDURE The student-athlete is to contact the Athletic Trainers or coach immediately and inform them of the injury. If the student-athlete needs to be referred to a physician or hospital a claim form will be completed. All medical bills will be sent directly to the student-athlete or to your home address. In some cases, the athletic trainer may get a copy of the bill, but in no case will the Athletic Department be the primary place the bill incurred to be sent. The student-athlete should submit all bills from providers of medical services to his/hers primary insurance carrier as soon as possible. Make sure to complete all claim forms and fulfill all other submission requirements of your primary carrier. They will do one of two things: • Honor the claim and pay all or a portion of the bills incurred. • Not honor the claim and send you a letter of denial. In the event your primary health care insurance does not cover the bills, please forward your carrier’s Explanation of Benefits (EOB) and any outstanding bills to: C.O.D. Athletic Training Once the athletic trainer receives the bills, they will be sent along with Athletic Department forms to the insurance carrier office for processing. If you receive a request from your insurance company or the College’s insurance company for additional information, please respond as quickly as possible, this will speed up the payment of the claim.
