TRANSCRIPTS
The Dental Hygiene program admits one class per
year. The following items must be on file in the Dental Hygiene coordinator's
office no later than Feb. 1 of the year in which the student applies.
OFFICIAL TRANSCRIPTS
1. Proof of high
school graduation or its equivalent (GED) is required. Please send
transcripts, high school and college, to the Records office, SRC 2015.
Allow at least three weeks for transcripts to arrive. Once transcripts
have arrived at C.O.D., come in to the Records office, SRC 2015, and
complete a Transcript Adviser Form.
Do NOT send transcripts to the Dental Hygiene office.
International transcripts from high school or college must be translated
into English (Cosmopolitan Translators, 53 W. Jackson Blvd., Suite
1260, Chicago, IL 60604, [312] 726-2610). Once these transcripts are
translated into English, the Educational Credential Evaluators, Inc.,
MUST evaluate them. The form for the Educational Credential Evaluators
is available from the College of DuPage Records office, SRC 2015 or at: www.ece.org.
2. You must complete prerequisite science and math courses
within the LAST FIVE YEARS with a cumulative GPA of 3.0 or better
on a 4.0 scale.
3. Placement testing in math and English is required of all
applicants who have not taken the prerequisite math and English prior
to applying to College of DuPage and/or the Dental Hygiene program.
Applicants MUST be tested through the Testing office, IC 100, or at
other C.O.D. sites that offer math and English testing.
4. All applicants are required to complete the PSB-HOAE exam (Health Occupations exam required by the Dental Hygiene program).
This test is offered through the Testing office, IC 100. Applicants
must contact the Registration office, (630) 942-2377, to reserve a
date and time. The (non-refundable) cost of this examination is $25.
Applicants can only take this test twice.
5. Student Planning Worksheet: This sheet helps you determine
which courses you need to take in order to fulfill the five-credit
Humanities course requirement along with the three-credit Contemporary
Life Skills course requirement. These are both required in order to
graduate from the College of DuPage Dental Hygiene program with an
Associate in Applied Science degree. |
