Hygiene Application Information

Program Requirements
It is the responsibility of the applicant to obtain and submit all
documents need to apply to the program to IC 1048A by the Feb.

• Audit Form
For transfer credit and high school transcript evaluation, applicants
must fill out a College of DuPage
Dental Hygiene Audit Form.
• Advising Sessions
Prior to application to the Dental Hygiene program, applicants MUST
attend a C.O.D. Dental Hygiene Advising Session. Upcoming dates/times
are available online.

you would like to attend.
• PSB-HOAE– Health Occupations Aptitude Exam
All applicants MUST take the PSB-HOAE prior to applying to the program.
Applicants are only allowed to take this exam twice, regardless
of the number of times they apply to the program. Contact the Registration
office, SRC 2048, (630) 942-2377, or check for PSB-HOAE dates online.
• Drug Testing
All students accepted into the dental hygiene program MUST
complete a drug test during the summer prior to entering the program.
A second drug test is required the summer prior to the student’s
second year in the program. The drug tests must be administered
at C.O.D. Health Services.
• Proof of Residency
Proof of residency in the form of a utility bill, proof of mortgage
or lease agreement are verified twice yearly.
